What it is about
Tapping into forbidden magic always has a cost—sometimes it's sanity, sometimes it's your soul… and sometimes it's your body. These mutations are the warped echoes of dark rituals, eldritch bargains, cursed bloodlines, or exposure to unstable magical forces. Some are grotesque, others strangely beautiful, but all mark the bearer as changed.
This supplement provides 100 unique mutations to surprise, enhance, or horrify your players. These can be used as consequences of spellcasting, effects of cursed artifacts, or gifts from ancient beings. Some grant power, others inflict terrible flaws, and many walk the line between both.
Use this table by rolling a d100 to determine what mutation a character has received—or choose manually to best fit your scene. These are tools to inspire, not restrictions. If a mutation doesn’t fit your world, feel free to re-flavor it or re-roll entirely.
Example: A warlock taps into their patron’s power during a desperate moment. The DM rolls a d100: 37 – Shadow-fused Limbs. Now, the warlock’s arms ripple with ever-moving darkness, offering new abilities but drawing the suspicion of others.
